- A program that supports participants to make lasting lifestyle changes that includes advice regarding healthy eating and involves moderate physical activity.
- Inclusion: For Medicare subsidy, patients must have a score of ≥ 15 on AUSDRISK, MBS item 713 or 717 (but must NOT have diabetes). Patients who do not meet this criteria may attend but will pay the full fee of $220.

Home Medicines Review (HMR)

Eastern Sydney Division of General Practice
A Home Medicine Review is an assessment in the home by an accredited pharmacist for people at risk of medicines misadventure . The pharmacist will recommend changes to improve medicine effectiveness, safety and adherence.
Inclusion: Patients living at home for whom quality use of medicines may be an issue or who may be at risk of medication related problems.
Frequency: Once every 12 months, or more frequently if there has been a change in patient’s condition or regimen.
Steps:
1) GP assesses need for HMR
2) Patient consent obtained and documented in notes
3) Referral form is sent to patient’s preferred community pharmacy
4) Accredited pharmacist conducts interview, usually in patient’s home
5) Pharmacist provides GP with a HMR report discussing issues arising
6) GP arranges a follow up visit with patient to discuss a medication management
plan (item 900).
A copy is given to the patient, one is forwarded to the pharmacist and one is retained.

Community Pharmacy
Patient’s should be referred to their local community pharmacy for a HMR. The community pharmacy will arrange for an accredited HMR pharmacist to contact the patient to arrange a visit, usually in the patient’s home.
