WHAT is the Lifestyle Modification Program?
- Provided by accredited health professionals (diabetes educators, exercise physiologists and dieticians) it involves 10 hours of small group education over six months.
- Patients learn about healthy eating, exercising and staying motivated.
- It involves moderate physical exercise and supports participants in making lasting changes.

WHO is eligible for the subsidy?
- If you have billed a 710, 713 or 717 and the patient has scored ≥15 on AUSDRISK (but DOES NOT have diabetes) they can attend the program for only $50.00 with UNSW Lifestyle Clinic and $10 with Diabetes Australia – NSW (full cost is $264.00).
- Health care and concession card holders receive the course for free.
HOW To Refer?
- Fill in the AUSDRISK and General Practitioner Referral Form
- PDF and Electronic versions of the AUSDRISK and the GP Referral Form are available below
- Call the Division on 9389 0874 for a Diabetes Unique Identifier (DUI) and fax the referral to the Division on 9387 4175 for processing.

Type 2 Diabetes Risk Evaluation Item 713 (updated 01 October 2008)
The Type 2 Diabetes Risk Evaluation (MBS Items 713) was released on 1 July 2008 as part of the 'COAG - reducing the risk of type 2 diabetes' initiative to address the significant growth in type 2 diabetes in Australia. This MBS item enables GPs to review patients aged 40 to 49 years (inclusive) who are at high risk of developing type 2 diabetes and instigate early interventions to assist in the prevention of type 2 diabetes.
